UPPER SCHOOL REQUIRED FORMS

All forms must be completed and returned to Severn School NO LATER THAN MONDAY, JULY 7. Students may not begin classes nor participate in Severn's program in any way - including sports - until all forms are completed and returned.

SEVERN SCHOOL FAMILY INFORMATION FORM
New for this year, this will all be done online! Verify and/or update your current contact information for our database and for the 2014-2015 Community Directory. Must be completed NO LATER THAN July 7, in order to have the Directory ready for distribution at the start of the new school year.

FINAL RECORD RELEASE 
Required for all new-to-Severn students. Please provide this signed form to the appropriate school personnel at your child’s current school. Receipt of final grades is a requirement for enrollment. 

PERMISSION for DISPENSING of OVER-the-COUNTER MEDICATION
Required for all students. One page.

ALL UPPER SCHOOL READ: This year’s Upper School Read  Day will take a different form, as students select a book to read from nearly 40 titles recommended by faculty. Students are allowed to list two choices from the many available; they will then be placed in a small “book club” with around ten other students. Students will know before they leave here in June which book they should read over the summer. In the fall, we will enjoy lunch with our groups and then a spirited discussion or activity revolving around the books. Everyone is encouraged to select a title they have not read before and to read the book rather than simply watch the movie. The choices include non-fiction, fiction, action, mystery, memoir, popular, and not-so-well-known titles. This reading requirement is in addition to the English department summer reading.

HEALTH INVENTORY & IMMUNIZATION CERTIFICATE
NEW Lower School students must complete the Maryland State Department of Education Health Inventory form, including the Immunization Certificate. Four pages. 

MARYLAND STATE IMMUNIZATION CERTIFICATE
Required for all returning prekindergarten and kindergarten students. Please note: ALL kindergarten students are now required to have TWO (2) doses of Varicella vaccine. Two pages.
